Yeah, 18.Nh4 seems to be premature, since it allows Black to force an exchange of queens with Qa4-f4. Better was 18.Bd5 to keep the pressure on (18...Qa4 19.Qd3). Also, 15.dxe5 might have been better than Bxf6 - e.g. 15...Nxd5 (15...dxe5?? loses to 16.Nxe5) 16.Qxd5 0–0 (16...dxe5 loses again to 17.Nxe5; 16...Be6 17.Qxb5) 17.exd6 cxd6 18.Qxd6.
