Perhaps 19...Qxd2 20.Rxd2 Nf5 seems to solve black's problems. After 21.Nc7+ Ke7 22.Nxa8 Rxa8, black has strong chances of holding because of his good knights protecting the past pawn.

Also, Karpov may have prepared either 19...Kf8 or 19...Qd8
